Job Requirements & Qualifications

  • Have graduated from an accredited Physical Therapist therapy program. (2021 grads welcome)
  • Possess a current state license (or obtaining in 2021)
  • Strong, disciplined work ethic
  • Dedication to quality patient care through the entire therapeutic process
  • Ability to communicate with a wide range of patient personalities
  • Excellent verbal and written communication skills
  • A strong sense of workplace professionalism and personal ethics
  • Demonstrated experience with evidence-backed treatment modalities

Care Coordinator

Are you organized and do you want to enjoy coming to work while making a difference in people’s lives? If you are personable, punctual, self-motivated, have good customer service skills, and are interested in becoming part of a great team, we would love to meet you.

We’re looking for a professional who believes that patients should be treated like family, and who understands the importance of compassionate service. You must be prepared for multitasking and are able to speak to patients about how to be in compliance with company policy and procedures.

Job Schedule: Full Time 40 hours

8:00am – 5:00pm Monday – Friday

Preferred Knowledge, Skills and Abilities:

  • Candidate must have the ability to interact with patients, families, coworkers, and employer in a professional manner; check-in and check-out patients efficiently; work in a fast-paced environment; prioritize tasks and remain organized in the completion of tasks; handle conflict in a professional manner;
  • Dependable and punctual and have good writing and communication skills.
  • Proficient in Microsoft Word, Excel and have a working knowledge of an iPad.
  • Ability to answer a multi-line phone.
  • The ability and willingness to treat all patients with the utmost kindness and consideration in the most trying situations.
  • Self-disciplined (does not require supervision) and follows through (you do what you say you are going to do without follow up needed)

Job Duties:

  • Greet patients/visitors; Upon greeting patients on arrival, you sign them in and obtain insurance information and any other necessary data we require from them
  • Scheduling appointments and data entry of patient information
  • Calling patients to remind them of appointments
  • Scanning, filing, emailing, faxing documents as needed
  • Answering and directing calls on a multi-line system
  • Making appointments
  • General office duties
  • Prepare and maintain patient’s EMR files by collection appropriate information and entering/filing correctly
  • Provide patients with intake and new patient forms as well as copies or our office policies and ensure paperwork is completed correctly.
